NTCA Health Plan Application Form

Good day Finley associates,

This is a follow-up to my email on 9-14-17 announcing the opportunity to receive a proposal for medical health insurance from NTCA, a Rural Broadband Association. Their plan will provide us with the following:

  • Lower your health insurance premium significantly and save you money
  • Lower deductibles
  • Lower co-pays

NTCA has a wide variety of benefit plans which offer better deductibles than we currently have at more competitive prices.

Here is the catch.

NTCA requires organizations like Finley to have all health plan eligible employees complete the attached Statement of Health Program/Group Health Program Form. You must complete ONE form for yourself as the applicant and include your spouse and any dependent children who are under age 26 prior to 1-31-2018 who you could enroll in the plan.

Again, their requirements are that ALL eligible employees complete the form – even if you do not believe you will enroll, you still need to fill out the form in order for Finley to be considered.

Please complete and return the application to your office manager as soon as possible but no later than Friday 10-13-17.  Please place your name on the outside of the sealed envelope when you return it to your Office Manager.  Placing the application in a sealed envelope with your name on it will allow us to track who has completed the application forms without seeing private health information. We can assure you this information will be handled in accordance with HIPAA privacy regulations.

AS A FUN WAY TO ENCOURAGE EVERYONE TO RETURN THE FORMS IN A TIMELY MANNER, ALL ASSOCIATES WHO SUBMIT THE APPLICATION BY 10-13-17 WILL BE ENTERED INTO A DRAWING FOR A FITBIT.

Finley does not take lightly the high cost each of us incur for medical insurance. By switching to the NTCA plan, the attached worksheet illustrates the savings to you and to Finley.  The plans we are considering have lower deductibles and better overall coverage.

This is why it is important we all follow through on this request.

Additionally, your local office manager has copies of the summary sheets for 2 plans we are considering with NTCA. Please contact your Office Manager if you would like to review that information.  The plans we are considering include  H.S.A. and P.P.O. options, employee only, employee child, employee spouse and family categories.   Coverage also includes all other plan provisions like Rx, chiropractor, PT, etc. benefits.
